What makes a good accessibility operations KPI

A useful KPI meets three criteria: it measures something the team can influence, it signals a problem early enough to act on, and it is interpretable without additional context. Many accessibility programs track metrics that fail one or more of these tests. "Number of documents remediated" measures output volume but says nothing about quality, timeliness, or user impact. It is a vanity metric that can increase while real program health declines.

Good KPIs connect operational behavior to business outcomes. "First-pass quality gate pass rate" tells you whether conversion work meets standards before handoff. "Mean time to resolve severity-1 support tickets" tells you how quickly user-impacting issues get fixed. "Defect escape rate" tells you how often quality gate failures reach the end user. Each of these metrics is actionable because a declining value points to a specific process that needs attention.

Limit your primary dashboard to eight to twelve KPIs. More than that creates information overload and dilutes attention. Group KPIs into four categories: intake health, delivery performance, quality outcomes, and support effectiveness. Each category should have two to three primary metrics that together provide a complete picture of that operational area.

Every KPI needs a named owner who is responsible for monitoring it, investigating anomalies, and initiating corrective action when thresholds are breached. A KPI without an owner is a number that nobody acts on.

Intake health metrics

Intake health metrics measure the quality and efficiency of the request pipeline before conversion work begins. The most important intake metric is rejection rate by reason code. A rising rejection rate signals upstream problems: unclear submission guidance, changing source document quality, or misaligned expectations about supported formats.

Track request-to-start cycle time, the duration between request submission and conversion work beginning. This metric captures queue wait time, which directly impacts user perception of service speed. Long request-to-start times indicate capacity constraints, prioritization problems, or inefficient intake processing.

Monitor intake volume trends by document class, priority tier, and submitter. Volume spikes in a specific document class may indicate an organizational event (new policy publication cycle, audit preparation) that requires proactive capacity allocation. Declining volume from a specific submitter may indicate dissatisfaction or workflow workaround.

Measure metadata completeness rate at submission. Incomplete metadata causes downstream delays as conversion specialists pause work to request missing information. A low completeness rate indicates that your submission form or intake guidance needs improvement.

Delivery performance metrics

Delivery performance metrics measure the speed and predictability of the conversion pipeline. The primary metric is end-to-end cycle time: the duration from request submission to accepted handoff. Track this metric overall and broken down by complexity tier, priority level, and document class.

SLA adherence rate measures the percentage of deliveries that meet their committed turnaround time. This is the most visible metric for stakeholders and the most likely to appear in vendor performance reviews. Track SLA adherence at the overall program level and per customer or department.

Throughput measures the volume of completed conversions per time period, typically weekly or monthly. Throughput trends, combined with cycle time trends, reveal capacity utilization. Rising throughput with stable cycle time indicates healthy growth. Rising throughput with increasing cycle time indicates capacity stress.

Track revision request rate, the percentage of deliveries that require post-handoff revisions before acceptance. A high revision rate indicates quality issues in the conversion or review process. A low but rising revision rate is an early warning that quality is beginning to slip before it reaches critical levels.

Quality outcome metrics

Quality outcome metrics measure the accessibility conformance and structural integrity of delivered documents. The most important quality metric is first-pass quality gate pass rate: the percentage of documents that pass all quality gates on the first attempt without requiring rework.

Track defect density by defect type: heading hierarchy errors, table structure issues, missing alt text, reading order problems, link integrity failures, and language declaration gaps. Defect density trends by type reveal which aspects of the conversion process need the most improvement and which training investments will have the highest return.

Defect escape rate measures the percentage of defects that are not caught by quality gates and are instead discovered by users, support teams, or external audits. This is the most critical quality metric because it measures the effectiveness of your entire quality system. A rising defect escape rate means your quality gates are becoming less effective, which demands immediate investigation.

Accessibility score trends, measured against WCAG 2.2 success criteria, provide a standardized quality benchmark. Track the average, median, and distribution of accessibility scores across all deliveries. A bimodal distribution (some documents scoring very high and others very low) indicates inconsistent conversion quality that role-specific training or process standardization can address.

Support effectiveness metrics

Support effectiveness metrics measure the responsiveness and resolution quality of post-handoff support operations. Mean time to acknowledge measures how quickly support tickets receive an initial response. Mean time to resolve measures total resolution duration by severity level.

Track ticket volume trends by issue type and originating document. Rising ticket volume overall may indicate program growth. Rising ticket volume per document indicates quality regression. Rising ticket volume concentrated in a specific issue type indicates a systematic process gap.

Measure reopened ticket rate: the percentage of resolved tickets that are reopened because the initial resolution was insufficient. A high reopen rate indicates that support is performing superficial fixes rather than addressing root causes, or that resolution quality standards are not clearly defined.

Customer satisfaction with support interactions, measured through post-resolution surveys, provides the user perspective that operational metrics alone cannot capture. A support team that resolves tickets quickly but leaves users frustrated is not performing effectively. Even a simple thumbs-up/thumbs-down rating after resolution provides actionable signal.

Reporting frequency and audience

Different audiences need different reporting cadence and detail levels. Operational team members need daily or real-time dashboard access to monitor queue health, active work status, and emerging issues. Program managers need weekly summary reports that highlight trend changes, SLA performance, and upcoming capacity challenges.

Leadership and executive stakeholders need monthly trend reports that connect operational KPIs to business outcomes: compliance posture, cost per document, program efficiency gains, and risk indicators. Monthly reports should include root-cause analysis for any significant metric changes and recommended actions.

External stakeholders (customers, auditors, procurement reviewers) need quarterly or on-demand performance summaries that demonstrate SLA adherence, quality gate effectiveness, and conformance outcomes. These reports should be structured for non-technical audiences and focus on results rather than process details.

Automate reporting wherever possible. Manual report assembly is time-consuming, error-prone, and delays distribution. If your KPI data lives in operational systems, build automated dashboards and scheduled report distribution so reporting is a byproduct of operations rather than an additional task.
